NELFT: Dementia Crisis Support Team

The Dementia Crisis Support Team works with adults over 18 years of age who have a diagnosed Dementia or a suspected Dementia. We cover Thurrock, Basildon & Brentwood residents. The aim of our team is to reduce a patient’s need to access A & E services and to avoid a hospital admission during a crisis period. We engage with the patient and / or carer in their own home or usual place of residence with rapid assessment, intervention, treatment and short term follow up for a maximum of six weeks.

We are a team made up of health professionals including Psychiatrists, Nurses and support workers all of whom have the relevant skills and experience. There is a nominated Social Care Dementia Lead / social worker from Thurrock area within the team who undertakes crisis work alongside the health professionals.

We engage with the patient and / or carer in their own home or usual place of residence with rapid assessment, intervention, treatment and short term follow up for a maximum of six weeks. This includes:

  • Assessment and support in response to a crisis or a potential deterioration in mental health 
  • Signpost patients who develop acute physical illnesses whilst under our care, by liaising with GP’s, community physical health services and other appropriate services.
  • A medication review and prescribing of mental health medication to prevent unplanned emergency admissions
    in conjunction with the GP
  • Support to maintain independence and safety to remain in your own home
  • Working in conjunction with other services including health teams, social services and voluntary organisations
